The start of the season (Sunday, April 15th) is less than three weeks away.
USA Rugby requires every child participating to fill out the attached waiver and questionnaire. Both forms must be submitted before your child receives their rugby kit. USA Rugby will not allow a child to participate this season until the waiver and the medical questionnaire have been submitted.
This Sunday, April 1st we will be distributing Kits and collecting forms.
We will be at Revere Park (2509 W Irving Park Rd) between 1:00 – 3:00 this Sunday, April 1st to distribute kits and collect forms. We are staggering times by age group, however, it is more important than you come to drop off forms and pick up your kit than to come at the slotted time.
1:00 – 1:30 U-7 Form Drop Off Only
1:30 – 2:00 U-9 Form Drop Off & Kit Distribution
2:00 – 2:30 U-11 Form Drop Off & Kit Distribution
2:30 – 3:00 U-15 Form Drop Off & Kit Distribution
If possible, please bring your child with you to ensure proper fit for their jersey.
